- Print the application form in the prescribed format.
- Complete all mandatory fields in the application form, attach a passport-sized photograph (signed if necessary), and include copies of required documents (self-attested if needed).
- Submit the filled and signed application form along with the documents to the District Social Welfare Officer, Superintendents at the Government Working Women’s Hostel, or the Extension Officer (Social Welfare).
