Application Process

Online Application via Common Service Center (CSC)

1. Gather all necessary documents, ensuring they are self-attested where required.
2. Visit the nearest Common Service Center (CSC) to complete the biometric verification process.
3. Fill out the application form, ensuring all mandatory fields are completed, and upload the required documents.
4. Request a receipt or acknowledgment from the CSC, making sure it includes the submission date, time, and a unique identification number if applicable.

Offline Application

1. Print the application form in the prescribed format.
2. Complete all mandatory fields in the application form, attach a passport-sized photograph (signed if necessary), and include copies of required documents (self-attested if needed).
3. Submit the filled and signed application form along with the documents to the District Social Welfare Officer, Superintendents at the Government Working Women’s Hostel, or the Extension Officer (Social Welfare).
4. Request a receipt or acknowledgment from the relevant authority upon submission, ensuring it contains the submission date, time, and a unique identification number if applicable.
