The Government of Uttar Pradesh has launched the “Surgical Grant for the Prevention of Disability” scheme, which is managed by the Department of Empowerment of Persons with Disabilities. This initiative aims to provide essential surgical assistance to economically disadvantaged individuals with disabilities, ensuring that they receive the necessary medical support to prevent further impairment.

The primary goal of this scheme is to extend surgical help to underprivileged individuals who are disabled, thus preventing potential disabilities. The initiative covers a wide range of surgical treatments aimed at various kinds of disabilities, including visual, auditory, and orthopedic impairments.
Financial Support Details
Eligible individuals can receive a financial grant of up to Rs. 6,00,000/- for the following types of surgeries:
- Surgery for Visually Impaired Individuals:
- Intraocular lens implant
- Cranioplasty
- Cranial repair
- Surgery for Hearing Impaired Individuals:
- Cochlear implant
- Tympanic membrane repair
- Mastoid surgery
- Surgery for Orthopedic Disabilities:
- S P Nail Operation
- Orthosis
- Artificial prosthesis
- External fixation
- Replacement implant
- Knee, hip, and ankle correction surgeries
- Shoulder, elbow, and wrist correction surgeries
- Post-polio corrective surgery
- Contracture repair
- Ligament repair
- Tendon transplant
- Soft tissue release surgery
- Contracture correction surgery
- Elizarov landing and corrective surgery
- Post-Leprosy Cured Disability:
- Reconstructive surgery for hand
- Reconstructive surgery for foot
Advantages of the Scheme
This scheme provides significant financial assistance to individuals, enabling them to access crucial medical interventions that may otherwise be unaffordable. The maximum grant amount of Rs. 6,00,000/- per eligible disabled person ensures that a substantial portion of surgical costs is covered.
Eligibility Requirements
To qualify for the benefits of the scheme, applicants must meet the following criteria:
- The annual income of the applicant or their guardian must not exceed Rs. 60,000/-.
- The applicant must be a citizen of India.
- The individual should be a permanent resident of Uttar Pradesh or have been a resident of the state for a minimum of five years.
- The applicant should not have any criminal charges against them.
Steps to Apply
Interested candidates can apply offline by following these steps:
- Download the application form from page number 18 of the official website.
- Complete the application form accurately.
- Attach the following documents to the application:
- A certificate of disability issued by a qualified medical authority.
- A certificate indicating annual income from all sources.
- Details of the estimated expenditure for the surgery, provided by the superintendent or in-charge of a State Hospital.
- Submit the completed application to the District Disabled Welfare Officer, along with a recommendation.
- The application, along with the recommendation, will be forwarded to the Director for further processing.
- In urgent cases, applications may be sent directly to the Director.

The following documents shall be attached to the application
(a). A certificate of disability granted by a competent medical authority.
(b). Certificate of annual income (from all sources).
(c). Estimated expenditure details of the surgery by the superintendent/in charge of the State Hospital.
Submit the application to the District Disabled Welfare Officer along with the recommendation through whom the application along with its recommendation will be sent to the Director.
In case of emergency, applications can be sent directly to the Director.
