• The individual must be an industrial worker or employee as defined under clause (C) of Section 2 of the Goa, Daman and Diu Labour Welfare Fund Act, 1986.
• Applicants should have completed a minimum of one year of service in their respective industrial establishment at the time of application, which may include broken periods of service.
• It is mandatory for applicants to have their eyes tested at State Hospitals, Goa Medical College, or by a certified Optician.
• Obtaining the lens number for the spectacles is required before applying.

• Step 2: Complete all necessary fields in the application form. Attach a passport-sized photograph if required, and include copies of all mandatory documents, ensuring they are self-attested where appropriate.
• Step 3: Submit the completed and signed application form along with the necessary documents through your employer to the Secretary, Goa Labour Welfare Board, located on the 4th floor of Shram Shakti Bhavan, Patto Centre, Panaji, Goa – 403 001.
