Online Application

To apply online, follow these steps:

3. Complete the form on the dashboard by filling in all necessary details, ensuring mandatory fields are marked with an asterisk (*).
4. Attach required documents, including identification proof and any additional certificates, ensuring clarity and compliance with format requirements.
5. Click the “Submit” button after completing the form and uploading documents. You will be redirected to the payment gateway to pay the application fee online.
6. Upon successful submission and payment, a registration number will be generated for your records.

Scheme Application Process

To apply for the benefits of the scheme, follow these steps:

Offline Application

For those preferring to apply offline, the registration process is as follows:

1. Visit the Labor Welfare Office during operational hours and request a hard copy of the application form.
2. Fill in all mandatory fields on the application form, attach a passport-sized photograph (signed across if necessary), and include copies of all required documents (self-attested if needed).
3. Submit the completed application form along with a wage slip or appointment letter from the employer as proof of completing 90 days of construction work. Alternatively, a certificate from a local authority can be provided to verify employment.
4. Upon submitting documents and payment, the applicant will receive an acknowledgment receipt confirming their registration.

Note:

• Applications must be submitted within the specified timeframe, if applicable.
• After three years, workers must visit the office to pay a renewal fee and provide evidence of 90 days of work each year to maintain eligibility for benefits.
• A one-time registration fee is required, which remains valid for three years.
