• The deceased must have been a registered member of the Puducherry Unorganised Labourers’ Welfare Society.
• The deceased should have been between the ages of 18 and 59 years during their lifetime.
• An annual subscription fee of ₹100 must have been paid by the deceased.
• A one-time registration fee of ₹25 should also have been paid by the deceased.

1. Visit the Office: Interested applicants should go to the Puducherry Unorganised Labourer’s Welfare Society located at No.1, Rue Sufferen, Puducherry, to collect the application form at no cost.
2. Fill the Application: Complete all required fields in the application form. Affix a passport-sized photograph (signed across if necessary) and attach copies of mandatory documents (self-attested where required).
3. Submit Form: Submit the completed application form along with the necessary documents to the designated authority.
